CardioOne is hiring a Virtual Care Manager to play a critical role in helping to stand up virtual care services. This role is responsible for providing critical remote support to patients in-between office visits with the goal of keeping them healthy, optimizing our ability to intervene quickly when they're not, and encouraging patients to take more control of their personal healthcare goals.
As a telephonic Maternity Nurse Care Manager, you will guide members through both routine and high-risk pregnancy experiences, working closely with a multidisciplinary team to deliver integrated remote care. You’ll connect directly with members, listen to their concerns, answer questions, and serve as a trusted advocate. This role requires strong clinical expertise in maternity care.
Support a high-need Medicaid population through proactive, whole-person care as a Bilingual RN Care Manager. The role involves delivering integrated medical, behavioral, and social support to individuals with complex needs and serving as the primary point of contact for a panel of high-risk members, contributing clinical expertise to build trust, address health disparities, and coordinate care across settings.
Reporting to the Supervisor of Care Management, this role involves providing case management to members, addressing their medical, social, psychological, physical, and spiritual needs. Responsibilities include developing, implementing, and monitoring care plans in collaboration with PCPs, caregivers, and team members to meet member needs in managed care.
